Fill out the Transcript Release Form located in the Guidance Office.

Former Students
A request for a transcript must be in writing, either by mail or by fax, and must include the following:

• Name
• Date of Birth
• Dates of Attendance and/or Graduation Date
• Full address where the transcripts are to be sent. If being picked up, please specify WILL PICK UP.
• A telephone number you can be reached at.
• Your signature
